Walla Walla nurse loses her license

OLYMPIA  — The Washington State Department of Health reports that a Walla Walla nurse has had her credential suspended immediately by the Nursing Commission.  Alexis Danielle Carr was placed on the long-term care abuse and neglect registry in 2020 by the Department of Social and Health Services.

The DOH reports that Carr, who was licensed in 2016, was employed at a skilled nursing facility the summer of 2018 when she removed some of the opioid painkiller tablets that were allotted for three patients at the facility. She did not document administering or destroying the 14 tablets that were unaccounted for.

Carr is barred form being employed to care for or have unsupervised access to vulnerable adults. She also has to hand over her credentials to the state. By utilizing the “How do I…” portion of the site, medical providers licensed after July 1998 may be searched for information of any disciplinary measures against them.
