NOTICE TO RESIDENTS You may soon receive a postcard in the mail requesting that you complete an income survey. This postcard will be coming from Portland State University for the benefit of Canyon City. • This survey is anonymous. • No personal information will be shared or used to identify you. • The information collected will only be used in summary form for program and funding purposes that support Canyon City. If we do not receive a response to the mailed survey, a volunteer may visit your residence to help gather the information in person. Thank you for your time and cooperation.
