Elgin Clinic Earns Tier 5 PCPCH Certification, Oregon’s Highest Standard for Primary Care

ELGIN – (Release from Grande Ronde Hospital) Grande Ronde Hospital and Clinics is proud to share that its Elgin Clinic has been awarded Tier 5 certification through Oregon’s Patient-Centered Primary Care Home (PCPCH) program—the highest level of recognition offered by the Oregon Health Authority. This elite certification is reserved for clinics that exceed the state’s most rigorous standards in providing accessible, team-based, and patient-centered care. Elgin Clinic now joins Grande Ronde’s Union Clinic, which earned the same honor in July, marking a powerful milestone for rural healthcare in Union County.

To qualify for Tier 5, clinics must meet all core PCPCH standards and demonstrate advanced performance in areas such as same-day access, behavioral health integration, proactive care planning, and equity-informed practices. Only a select number of clinics across Oregon achieve this level of distinction.
