DAS contractor makes mailing error

SALEM – The Oregon Department of Administrative Services (DAS) has learned that a mailing error regarding the tax form 1099-G affected over 30,000 people in Oregon. The error impacted 32,960 out of 122,245 mailings. Some people may not have received a 1099-G and others may have gotten a form that did not belong to them.
According to the state of Oregon, a machine error caused some of the forms to go into envelopes already containing a 1099-G. DAS was first made aware of this error on Wednesday of last week, during the ongoing printing and mailing process for the 1099-G forms and called for an immediate halt.
“This was an issue caused by a contractor,” the state of Oregon said in a release, “but is ultimately the Department of Administrative Services’ responsibility. We take ownership of this error and are correcting this mistake immediately.”
The Department of Administrative Services will finish mailing and will re-send the tax forms to the entire subset of over 30,000 by Jan. 31.
What information was involved?
The information contained in the 1099-G forms included:
- Recipient’s name and address
- Recipient’s TIN, also known as the last four digits of the social security number.
- Amount of unemployment compensation
- Amount of federal income tax withheld
- Amount of state income tax withheld
The forms are filed by federal, state, or local governments if they made payments of unemployment compensation; state or local income tax refunds, credits, or offsets; reemployment trade adjustment assistance payments; taxable grants; or agricultural payments. They also file the form if they received payments on a Commodity Credit Corporation loan.
